Accident Summary:
AIRCRAFT 66-16671 HAD COMPLETED ITS THIRD EXTRACTION AND WAS ON FINAL APPROACH TO A FIRE SUPPORT BASE WITH FOURTEEN AMERICAN TROOPS ON BOARD. THE TWO PRIOR EXTRACTIONS HAD BEEN COMPLETED WITHOUT INCIDENT. THE PILOT HAD ELECTED TO ALTER THE APPROACH HEADING FROM THE LAST TWO APPROACHES IN ORDER TO TAKE ADVANTAGE OF THE WIND WHICH AT THAT TIME WAS FROM 120 DEGREES AT 5 KNOTS. FINAL APPROACH HEADING WAS APPROXIMATELY 150 DEGREES AND DIRECTLY OVER THE RIVER. AT AN ALTITUDE OF APPROXIMATELY 20 TO 25 FEET AND 20 TO 25 KNOTS FORWARD AIRSPEED AND JUST PRIOR TO REACHING THE LANDING PAD, THE AIRCRAFT LOST POWER AND DESCENDED RAPIDLY IN AN ATTITUDE DESCRIBED BY THE PILOT AS TAIL LOW AND NOSE RIGHT. THE PILOT APPLIED ADDITIONAL COLLECTIVE PITCH TO CUSHION BUT THE AIRCRAFT LANDED HARD AGAINST THE RIVER BANK AND SHORT OF THE PAD. AT THE SAME TIME THE TAIL ROTOR CONTACTED THE WATER AND SEPARATED FROM THE AIRCRAFT ALONG WITH THE 90 DEGREE GEAR BOX. THE AIRCRAFT THEN BOUNCED AND STARTED SPINNING TO THE RIGHT IN A NOSE LOW ATTITUDE TURNING APPROXIMATELY 270 DEGREES. THE PILOT ATTEMPTED A HOVERING AUTOROTATION AND THE AIRCRAFT SETTLED LEVEL BUT WAS TOO NEAR THE RIVERS' EDGE. THE GROUND UNDER THE LEFT SKID GAVE WAY AND THE AIRCRAFT SLID INTO THE WATER. THE MAIN ROTOR THEN CONTACTED THE WATER AT THE NINE O'CLOCK POSITION WITH SUCH FORCE THE MAIN TRANSMISSION SHEARED AT ITS ATTACHING BOLTS TO THE MAIN CASE. THE MAIN TRANSMISSION AND THE MAIN ROTOR WERE THROWN FORWARD AND CAME TO REST IN THE RIVER APPROXIMATELY '4 METERS TO THE LEFT FRONT OF THE AIRCRAFT. THE AIRCRAFT WAS NOW RESTING ON ITS LEFT SIDE PARTIALLY SUBMERGED IN FIVE FEET OF WATER. THE ENGINE CONTINUED TO RUN FOR NEARLY 30 MINUTES DESPITE EFFORTS MADE IN THE COCKPIT. WHEN MAINTENANCE PERSONNEL ARRIVED THE THROTTLE LINKAGE WAS DISCONNECTED AND THE ENGINE SHUT DOWN.\\
